If after having accessed documents held by us you identify your personal information is inaccurate, incomplete, out-of-date or misleading you may make application for correction or amendment of any part of the information.

You can only amend those parts of a government document that contain your personal information.

Any such application must:

  • state you have seen the document or have a copy of it
  • provide enough information to identify the document
  • provide an address (post or email)
  • specify the information you want amended
  • explain how the information is inaccurate, incomplete, out-of-date or misleading
  • suggest how you want the information changed
  • complete the Information Privacy Personal Information Amendment Application Form
  • provide evidence of your identity.

Reviewing a decision

If you are dissatisfied or "aggrieved" with our decision made under the Right to Information Act 2009 or the Information Privacy Act 2009 regarding access or amendment to documents you can apply to our agency for an internal review of its decision, or alternately apply to the Information Commissioner for an external review of the decision.

To apply for an internal review of a decision you must apply in writing within 20 business days of receiving the decision.

Your internal review application will then be referred to the Manager, Right to Information and Information Privacy Unit for decision.
